Raloxifene (brand name Evista) is a second-generation selective estrogen receptor modulator (SERM) that is significantly more breast-tissue-selective than Nolvadex. While it is less commonly used for full PCT, it is considered the single most effective pharmaceutical option for reversing existing gynecomastia. Raloxifene has stronger antagonist activity at breast ER-alpha receptors than tamoxifen, with clinical studies showing gynecomastia reduction rates of 80-90% in pubertal cases. For steroid users, it is the go-to SERM when the primary concern is gyno reversal rather than HPTA restart.
Notes
Raloxifene was FDA-approved in 1997 for osteoporosis prevention/treatment and later for breast cancer risk reduction in postmenopausal women. Its off-label use for gynecomastia reversal in males has strong clinical evidence from studies on pubertal gynecomastia. For AAS users, it fills a specific niche: it is the best SERM for breast tissue concerns but should be combined with a stronger HPTA-stimulating SERM (Nolvadex or Enclomiphene) for actual testosterone recovery.
Raloxifene is a benzothiophene-derived SERM that acts as a potent estrogen receptor antagonist specifically in breast tissue. It binds ER-alpha and ER-beta in mammary tissue with high affinity, blocking estradiol-mediated cellular proliferation. In breast tissue, the raloxifene-ER complex recruits co-repressors rather than co-activators, shutting down estrogen-responsive gene transcription. This is what makes it superior to tamoxifen for gyno — it has a stronger anti-estrogenic effect at the breast. In bone tissue, it acts as an estrogen agonist, preserving bone mineral density. In the hypothalamus, it has weaker anti-estrogenic activity than tamoxifen or clomiphene, which makes it less effective for pure PCT HPTA stimulation.
Oral bioavailability is approximately 2% due to extensive first-pass glucuronidation, but clinical efficacy is well-established at standard doses. Peak plasma concentration in 0.5-6 hours. Highly protein-bound (>95%). Extensively metabolized by hepatic glucuronidation (not CYP-dependent — so no CYP2D6 metabolism concerns unlike tamoxifen). Half-life of 27-32 hours supports once-daily dosing. Enterohepatic recycling contributes to sustained plasma levels.
Typical Dose
Gynecomastia reversal: 60mg/day for 3-6 months. Gynecomastia prevention on-cycle: 30-60mg/day. PCT (less common use): 60mg/day for 4-6 weeks. Gyno flare-up acute treatment: 120mg/day for 10 days, then reduce to 60mg/day.
Frequency
Once daily. Consistent timing recommended for stable blood levels.
Administration
Oral tablet, taken with or without food.
Half-Life
27-32 hours (active metabolite: raloxifene-4'-glucuronide)
Contraindications
Raloxifene is not the first-choice standalone PCT compound because it is weaker at stimulating LH/FSH compared to Nolvadex or Enclomiphene. However, it can be added to a standard PCT protocol when the user has active gynecomastia that needs reversal. Recommended combo: Nolvadex or Enclomiphene for HPTA recovery + Raloxifene 60mg/day specifically targeting gyno tissue. Run the raloxifene for 3-6 months even if PCT is only 4-6 weeks.
