Letrozole is the most potent aromatase inhibitor available, capable of reducing estradiol levels by 97-99% at standard clinical doses. It is a non-steroidal, reversible third-generation AI that binds aromatase with higher affinity than anastrozole. In the bodybuilding community, letrozole is considered the "nuclear option" for estrogen control — reserved for emergency situations like rapidly developing gynecomastia, severely elevated E2, or highly aromatizing cycles that do not respond to lower-potency AIs. Its extreme potency makes it easy to crash estrogen, so it requires careful dosing.
Notes
Letrozole was FDA-approved in 1997 for breast cancer treatment. It is the most potent commercially available aromatase inhibitor. In the bodybuilding context, it should be viewed as an emergency tool, not a routine estrogen management compound. The trend among evidence-based coaches is to move away from routine AI use entirely, instead using lower AAS doses and SERMs for gyno protection while allowing estrogen to remain in a healthy range for cardiovascular, bone, brain, and joint health.
Letrozole competitively and reversibly binds to the heme group of the aromatase enzyme (CYP19A1) with extremely high affinity — approximately 170 times more potent than aminoglutethimide and 2-5 times more potent than anastrozole. By occupying the active site, it prevents the aromatization of androgens to estrogens. At 2.5mg/day (clinical dose), it suppresses whole-body aromatization by 97-99% and plasma estradiol to nearly undetectable levels. Because the binding is reversible, estrogen rebound occurs when the drug is discontinued — potentially more severely than with anastrozole due to the greater suppression.
Oral bioavailability is approximately 99.9%. Rapidly absorbed with peak plasma levels in 1-2 hours. Minimally protein-bound (60%). Metabolized hepatically via CYP3A4 and CYP2A6 to an inactive carbinol metabolite. Terminal half-life of approximately 2-4 days. Reaches steady state in 2-6 weeks at constant dosing, but at bodybuilding doses (intermittent, lower), functional steady state is reached faster. Excreted primarily in urine.
Typical Dose
Emergency gyno flare: 2.5mg/day for 7-10 days, then taper to 0.5mg EOD. On-cycle estrogen control (heavy aromatizers): 0.25-0.5mg every other day. Standard on-cycle use: 0.25mg every other day or twice weekly. Pre-contest/drying: 0.25-0.5mg EOD for final 2-4 weeks.
Frequency
Every other day (EOD) at minimum. Twice weekly (E3.5D) for maintenance. Only use daily dosing for acute gyno emergencies and limit to 7-10 days.
Administration
Oral tablet, taken with or without food. Available in 2.5mg tablets — bodybuilders typically use fractions.
Half-Life
2-4 days (approximately 48 hours)
Contraindications
Letrozole should NEVER be used during PCT. Its extreme potency will suppress estrogen to levels incompatible with HPTA recovery. Even more than anastrozole, letrozole during PCT will prevent LH and testosterone restoration. Discontinue all letrozole before starting PCT and allow E2 to normalize.
