Tadalafil is a long-acting phosphodiesterase type 5 (PDE5) inhibitor primarily used for erectile dysfunction and benign prostatic hyperplasia. In the bodybuilding and PED community, it serves double duty: improving sexual function (which is commonly impaired during and after steroid cycles) and providing performance benefits through enhanced blood flow, vasodilation, and improved muscle pumps. Its 36-hour half-life makes it ideal for daily low-dose use, earning it the nickname "The Weekend Pill." Many AAS users run low-dose tadalafil throughout their cycles as a quality-of-life enhancement.
Notes
Tadalafil was FDA-approved in 2003 (as Cialis) for erectile dysfunction and later for BPH and pulmonary arterial hypertension (as Adcirca at 40mg). Its long duration of action and lack of food interactions make it the preferred PDE5 inhibitor for daily use in the bodybuilding community. At 2.5-5mg daily, the side effect profile is very mild and the quality-of-life improvements are significant, especially during cycles that may impair sexual function (19-nors, heavy suppressive stacks).
Tadalafil selectively inhibits phosphodiesterase type 5 (PDE5), the enzyme responsible for degrading cyclic guanosine monophosphate (cGMP) in smooth muscle cells. cGMP is the second messenger that mediates nitric oxide (NO)-dependent vasodilation. By inhibiting PDE5, tadalafil allows cGMP to accumulate, causing prolonged smooth muscle relaxation and vasodilation in multiple vascular beds — including penile corpus cavernosum (erectile function), pulmonary vasculature, skeletal muscle vasculature (improved pumps), and prostatic/bladder neck smooth muscle (BPH relief). Unlike sildenafil and vardenafil, tadalafil also mildly inhibits PDE11, which is expressed in skeletal muscle, prostate, and testes.
Oral bioavailability is approximately 36% (independent of food intake — major advantage over sildenafil). Peak plasma concentration in 2 hours (range 0.5-6 hours). Protein binding approximately 94%. Metabolized primarily by CYP3A4 to a catechol metabolite (methylcatechol glucuronide) that is at least 13,000-fold less potent for PDE5. Terminal half-life of 17.5 hours, but pharmacological effects persist 24-36 hours. Reaches steady state in approximately 5 days at daily dosing. Excreted primarily in feces (61%) and urine (36%).
Typical Dose
Daily low-dose (general wellness/pumps): 2.5-5mg/day. As-needed for sexual performance: 10-20mg taken 30-60 minutes before activity. On-cycle standard: 5mg/day. Blood pressure support: 2.5-5mg/day. Pre-workout (pump enhancement): 10mg taken 1-2 hours before training.
Frequency
Daily dosing at 2.5-5mg is the most common protocol for AAS users. As-needed dosing of 10-20mg provides 24-36 hours of effect.
Administration
Oral tablet, taken with or without food (no significant food interaction unlike sildenafil).
Half-Life
17.5 hours (clinically described as up to 36 hours of effect due to active metabolites)
